ADOP-HAD

ADOP-HAD est un outil d'aide à la décision d'orientation des patients en hospitalisation à domicile (HAD). Son objectif est d'identifier les patients potentiellement éligibles à une HAD, le plus en amont possible afin d'anticiper leur orientation.

Cet outil est à destination des médecins prescripteurs d'HAD : médecins hospitaliers et médecins de ville.

Il peut être utilisé pour tous les patients quelles que soient leurs caractéristiques et leur provenance (établissement de santé, domicile, structure sociale ou médico-sociale avec hébergement).

ADOPT-HAD is a tool to aid policy decision of patients in hospital at home (HAD). Its objective is to identify potential patients eligible for HAD, the earliest possible stage in order to anticipate their orientation.

This tool is intended for prescribers HAH: hospital doctors and general practitioners.

It can be used for all patients regardless of their characteristics and origin (health facility, home, social structure or medico-social with accommodation).
